نقطه عطف میلهگذاری بهصورت نوین به جرارد کونچر برمیگردد. وی در سال 1940در کشور آلمان، میلهگذاری نوین را معرفی کرد و به دو اصل کلی اعتقاد داشت: فیکساسیون پایدار و فیکساسیون شکستگیها بدون بازکردن محل شکستگی. در ابتدا از میلههایی ابتدایی با نقاط ضعف متعدد و تنها در شکستگیهای تنه استخوانهای بلند از میلهگذاری استفاده میشد. به مرور زمان، تغییرات تاثیرگذاری در طراحی و کارگذاری میلههای استخوانی ایجاد گردید. هدف از این مقاله مروری بر تکامل طراحی میلهها و اصول ساده ولی بسیار مهم میلهگذاری استخوانهای بلند میباشد که در صورت عدم رعایت همین اصول ساده توسط جراحان، باعث عدم رسیدن به فیکساسیون دلخواه میشود.

صفحات 228-235مقدمهتعیین ارتباط تعادل و حس عمقی مفصل زانو در تکواندوکاران اهمیت گنجاندن تمرینات حس عمقی در برنامه تمرینی تکواندوکاران را نشان میدهد و میتواند بهعنوان یکی از مولفههایی که منجر به ارتقا سطح حرفهای ورزشکاران آماتور و حرفهای میگردد مورد توجه ورزشکاران و مربیان رشته ورزشی تکواندو قرار گیرد. لذا هدف از پژوهش حاضر مقایسه وضعیت حس عمقی وضعیت مفصل زانو و تعادل در تکواندوکاران حرفهای و آماتور بود.
مواد و روشهاروش پژوهش حاضر مقطعی و مقایسهای بود. نمونه پژوهش حاضر دو گروه 40 نفری از تکواندو کاران حرفهای و آماتور 18 تا 30 ساله بود. وضعیت فرد از نظر حرفهای بودن (حرفهای و آماتور) متغیر مستقل در نظر گرفته شد و میانگین خطای بازسازی زاویه هدف زانو در سه زوایه 30، 60 و 90 درجه و تعادل متغیرهای وابسته بودند که به ترتیب توسط ژیروسکوپ و دستگاه تعادل سنج بایودکس اندازهگیری شدند. از آزمون تی تست مستقل و معادل ناپارامتریک آن یو مان- ویتنی (در صورت برقرار نبودن پیش فرضهای آزمون پارامتریک) برای مقایسه متغیرهای مورد مطالعه بین دو گروه استفاده شد.
یافتههانتایج نشان داد که بین حس وضعیت مفصل زانو در زوایههای 30 (0.001p=)، 60 (0.001p=)و 90 (0.008p=) درجه و نیز تعادل (0.001p=) در تکواندوکاران حرفهای و آماتور تفاوت معنیداری وجود دارد و گروه حرفهای نسبت به افراد آماتور نتایج بهتری را کسب کردهاند.
نتیجه گیریباتوجه به یافتههای پژوهش نتیجهگیری میشود که انجام تمرینات تکواندو بهصورت حرفهای میتواند حس عمقی مفاصل و تعادل را با افزایش عملکرد جسمانی بهبود ببخشد و این موارد از نکات مهم تفاوت بین ورزشکاران حرفهای نسبت به تکواندکاران آماتور میباشد.کلیدواژگان: مفصل زانو، حس عمقی، پایداری پاسچر، تکواندو -
صفحات 236-242پیش زمینهامروز در خصوص انتخاب آرتروپلاستی در درمان شکستگی با جابجایی گردن فمور در افراد مسن اتفاق نظر وجود دارد. رویکردهای شایع جراحی در پشت یا کناری (پوستریور و لاترال) میباشند. استفاده از رویکرد جلو (دایرکت انتریور) در بسیاری مراکز دنیا محبوبیت پیدا کرده است. با درنظر گرفتن امتیازهای متعدد این رویکرد مطالعهای برای بررسی نتایج آن در افراد مسنی که در مرکز ما تحت عمل همیآرتروپلاستی با رویکرد «مستقیما جلو» قرار گرفته بودند انجام پذیرفت.مواد و روشهاتمام موارد شکستگی گردن ران که در مرکز آموزشی درمانی امام خمینی تهران در فاصله سه سال 2010 تا 2013 تحت عمل همیآرتروپلاستی با رویکرد «مستقیما جلو» قرار گرفته بودند از سیستم پروتکل آیندهنگر آن مرکز استخراج شدند - اطلاعات قبل، ضمن و بعد از عمل که همگی با برنامهریزی قبلی در سیستم موجود بودند جمعآوری شدند. همچنین اطلاعات پیگیریهای بیماران در درمانگاه در فواصل معین که همگی ذخیره شده بودند در آورده شدند و آنالیز شدند.یافتهها45 بیمار شامل 16 مرد و 29 زن با متوسط سن 73.4 در مطالعه شرکت داده شدند. بیماران پیگیری متوسط 76 ماه داشتند. هیچ یک از عوارض معمول از قبیل مشکلات زخم و عفونت عمقی و دررفتگی در این بیماران دیده نشد. امتیاز هاریس بیماران عدد 87 (95-76) بود.نتیجهگیریبه دلیل سن بالای اکثر افرادی که به شکستگی گردن فمور دچار میشدند، درمان همی آرتروپلاستی با رویکرد «مستقیما جلو» امتیازهای متعددی دارد و در این گروه از بیماران قویا توصیه میشود.کلیدواژگان: شکستگی گردن ران، همی ارتروپلاستی، اپروچ دایرکت انتریور

صفحات 243-250پیش زمینهناحیه دیستال ران شایع ترین مکان بروز تومور سلول غول آسا است. روش درمان جراحی شامل کورتاژ تومور و سیمان گذاری است. در حالیکه شکستگی یکی از معضلات پس از جراحی است، معیار یا داده بیومکانیکی دقیقی برای مشخص کردن بیمارانی که در معرض خطر بالای شکستگی قرار دارند وجود ندارد.مواد و روش هابا توجه به ارتباط تنگاتنگ بین کاهش در استحکام استخوان و خطر شکستگی آن، در این تحقیق با استفاده از روش اجزای محدود بر مبنای تصاویر برش نگاری کمی کامپیوتری، و با در نظر گرفتن مکانیک سطح مشترک سیمان و استخوان، علاوه بر لحاظ خواص ناهمگن و رفتار استخوان پس از رسیدن به تنش تسلیم، به بررسی استحکام استخوان پس از جراحی پرداخته میشود. صحت و دقت مدل های اجزای محدود در پیش بینی استحکام استخوان با مقایسه نتایج آنها با داده های آزمایشگاهی روی 14 نمونه انسانی و روی یک گروه داده مستقل به کمک آزمون های آماری ارزیابی می شود.یافته هاازمون t زوجی تفاوت معناداری بین بار شکست محاسبه شده از تحلیل اجزای محدود و ثبت شده در آزمایشگاه را نشان نداده است. همچنین شیب بدست آمده در رابطه خطی بین دو بار شکست، محاسبه شده توسط روش اجزای محدود و ثبت شده در آزمون مکانیکی آزمایشگاهی، از تحلیل رگرسیون خطی تفاوت معناداری را از 1 نشان نداده است.نتیجه گیرینتایج حاصل، توانایی روش اجزای محدود بر مبنای تصاویر برش نگاری کمی را در پیش بینی استحکام استخوان دیستال ران پس از جراحی تومور سلول غول آسا نشان میدهد. این مدل ها همچنین میتوانند برای بررسی بیشتر جنبه های نامشخص مسائل سیمان-استخوان مورد استفاده قرار بگیرند.کلیدواژگان: برش نگاری کمی کامپیوتری، روش اجزای محدود، استحکام استخوان دیستال ران، تومور سلول غول آسا، آزمون مکانیکی برون تنی

صفحات 251-255پیش زمینهرایج ترین روش درمان برای تالوس عمودی جراحی گسترده بافت نرم است. یک روش با حداقل تهاجم بر پایه قالب گیری سریالی یا پشت سر هم «دوب» (پونستی معکوس) 15 سال پیش معرفی شد و نتایج قابل توجهی به دست آورد. هدف از این مطالعه بررسی و مقایسه نتایج میان مدت قالب گیری با روش گچگیری است.
مواد و روشها12 بیمار مبتلا به تالوس عمودی در یک مطالعه آیندهنگر قرار گرفتند، 6 بیمار با 10 فوت مبتلا به پونستی معکوس و 6 نفر دیگر با 10 فوت با جراحی بافت نرم خارجی تحت درمان قرار گرفتند. در یک پیگیری 18 تا 24 ماهه، نتایج با استفاده از محدوده حرکت مچ پا، پرسشنامه اکسفورد و اندازهگیریهای رادیوگرافی مقایسه شدند.
یافتههامیانگین حرکات مچ پا در دو گروه قابل مقایسه بود (0.35 = p)؛ نمره پرسشنامه اکسفورد بین دو گروه به لحاظ آماری تفاوت معناداری نداشت (p = 0.876).با توجه به نتایج قابل مقایسه بین جراحی گسترده و گچگیری «پونستی معکوس» در این مطالعه میان مدت، تکنیک قالبگیری به دلیل ماهیت و طبیعت غیرتهاجمی این روش درمانی توصیه میشود.

صفحات 256-260پیش زمینهیکی از مشکلات شایع و علل مهم مراجعه به مراکز درمانی آسیبهای ارتوپدی است که عوامل مختلفی چون آسیبهای خود به خودی، برخورد با وسایل نقلیه، در محیط کار و یا هنگام ورزش در آن نقش دارد. هدف از پژوهش حاضر تعیین فراوانی و علل بروز آسیبهای ارتوپدی است.
مواد و روشهاکلیه بیماران مراجعه کننده به اورژانس بیمارستان آموزشی اختر استان تهران در سال 94 نمونه آماری پژوهش حاضر است که با روش پرسشنامه ثبت آسیب توسط پرستار بهصورت تصادفی جمعآوری گردید. از آمار توصیفی مانند فراوانی، میانگین و انحراف استاندارد برای بررسی شیوع آسیب و بیان ویژگیهای نمونه آماری استفاده شد. از آمار استنباطی خطی دو جهت مقایسه بین میزان شیوع آسیب در نواحی مختلف بدن استفاده شد.
یافتههانتایج پژوهش حاضر بیانگر آن بود که آسیبهای اندام تحتانی بیشتر از نواحی دیگر است و آسیبهای اندام فوقانی در رتبه دوم است. بین جنسیت و علت بروز حادثه سطح معنیداری وجود دارد (P<0.03). همچنین بین جنسیت و میزان بروز آسیب به ترتیب در اندامهای تحتانی چپ (P<0.001) و راست (P<0.049) و اندام فوقانی راست (P<0.001) و چپ (P<0.001) سطح معنیداری وجود دارد. بروز آسیب در میان مردان بیش از زنان و همچنین مچ پا شایعترین ناحیه آسیب دیده در اندام تحتانی چپ و راست و مچ دست شایعترین ناحیه در اندام فوقانی راست و چپ بود.
نتیجهگیریباتوجه به نتایج حاصل از این بررسی میتوان بیان کرد علت اصلی آسیبهای ارتوپدی مراجعین به بیمارستان اختر استان تهران حوادث سواره و پیاده است و بیشترین مصدومین در بازه سنی 20 تا 40 سال میباشند.کلیدواژگان: آسیبهای ارتوپدی، شکستگی، اورژانس -
فیکساسیون شکستگی استخوان هیومروس پایین پروتز شانه با پلاک قفل شونده مخصوص انتهای فمور (گزارش 3 مورد)صفحات 261-264شکستگیهای «پریپروستتیک» اطراف شانه، عارضه ای نادر و در عین حال مشکل برای درمان است. این نوع شکستگی را در 3% موارد تعویض مفصل شانه میتوان دید. این مقاله تکنیک جدیدی را گزارش میکند که با استفاده از پلاک قفل شونده مخصوص انتهای فمور شکستگی اطراف مفصل مصنوعی (پریپروستتیک) را درمان کرده است. 3 بیمار بین نوامبر 2008 تا ژوئیه 2016 با شکستگی «پریپروستتیک» استخوان هیومروس با جراحی باز و فیکساسیون با چنین پلاکی درمان شدند. هر سه بیمار تا جوش خوردن کامل براساس رادیوگرافی پیگیری داشتهاند. شکستگیها بین 1 تا 5/7 سال (متوسط 2/5 سال) پس از پروتز شانه بوقوع پیوسته بودند.
دو نوع شکستگی به عنوان رایت کافیلد نوع C و یک نوع شکستگی بعنوان رایت کافیلد نوع B طبقه بندی شدند. جراحی تثبیتی با میانگین 6 روز پس از شکستگی انجام شد. همه با یک پلاک قفلشونده با زاویه ثابت درمان شدند. جوشخوردگی هر سه بیمار با استفاده از رادیوگرافی، میانگین 11.8 ماه (در محدوده 3 تا 28.5 ماه) تایید شد. هیچ بیمار نیازی به جراحی دوباره نداشت. بدست آوردن تثبیت کافی استخوان در اطراف شکستگیهای استخوانی پریپروستتیک، چالش برانگیز باقی مانده است. استفاده از یک پلاک قفلشونده ممکن است در بیماران با پروتز با تنه طولانی مفید باشد که نیازمند افزایش تثبیت دیستال است.

The history of the new nailing method dates back to Gerhard Küntscher, who introduced this concept in Germany in 1940. He believed in two general principles: Stable fixation and fixation of fractures without opening the fracture site. In the beginning, primary nails with various weaknesses were only used in fractures of femur. Over time, effective changes were made in the design and insertion of bone nails. This article aimed to review the evolution of the design of nails and simple but significantly important principles of inserting nails in long bones. In any case, lack of adherence to these simple principles by surgeons leads to lack of desirable fixation.

Pages 228-235BackgroundDetermining the association between knee joint proprioception and stability in Taekwondo players shows the importance of incorporating proprioception exercises in the Taekwondo training program. In addition, it can be considered by athletes and Taekwondo coaches as one of the components that promote the professional level of performance. Therefore, this study aimed to compare the knee proprioception and postural stability between Iranian amateur and professional Taekwondo players.Materials & MethodsThis cross-sectional comparison study was performed on 40 Taekwondo players who were randomly divided into two amateur and professional groups. The level of players (professional and amateur) was considered as the independent variable, whereas the mean error of target angle reconstruction at 30, 60, and 90 degrees and postural stability were recognized as the dependent variables. Knee joint proprioception and postural stability were assessed using a gyroscope and Biodex stability system, respectively. Data analysis was performed using independent t-test, and its non-parametric equivalent, Mann-Whitney U (in case of lack of establishment of the assumptions of parametric test).ResultsA significant difference was found between amateur and professional groups regarding proprioception at 30 (P=0.001), 60 (P=0.001) and 90 (P=0.008) degrees as well as postural stability (P=0.001), where the professional group obtained better results, compared to the other group.ConclusionAccording to the results of the study, performing professional Taekwondo techniques can positively affect joint proprioception and postural stability by increasing physical performance. Pages 236-242IntroductionNowadays, there is consensus regarding arthroplasty as the preferred treatment method for displaced femoral neck fracture among the elderly population. Although posterior and lateral surgical approaches are the most commonly used approaches for hip arthroplasty, direct anterior approach (DAA) has become popular in many centres. Considering the many advantages of DAA, this study aimed to study the results of hemiarthroplasty via DAA for femoral hip fracture in the elderly people in our centre.
MethodsAll the femoral neck fractures that had received hemiarthroplasty via DAA between January 2010 and January 2013 in a teaching hospital were extracted from the database. The pre, intra, and post-operative information and clinical follow-up data were all collected and analysed. The Harris Hip Score and physical exam was used for functional assessment.
Results45 patients (16 males and 29 females) with a mean age of 73.4 years and a mean follow-up of 76 months entered the study. There was no complication such as dislocation, wound problem or deep infection. The final mean Harris Hip Score was 87 (76-95).
Conclusiongiven that the patients with femoral neck fractures are often old, hemiarthroplasty treatment via direct anterior approach (DAA), has multiple advantages and is recommended for this population.Keywords: Femoral Neck Fracture, Hemiarthroplasty, Direct Anterior Approach -
Pages 243-250BackgroundDistal femur is the most frequently affected site by giant cell tumor (GCT). Surgery including tumor curettage and defect reconstruction with bone cement is a widely-used treatment. While post-operative fracture is a common complication, no criteria and/or biomechanical data are available to identify patients at high risk of fracture.Methods and materialsSince there is a strong correlation between bone strength and its fracture risk, in this study, bone strength following GCT surgery was predicted using quantitative computed tomography (QCT)-based finite element method (FEM). Mechanical properties of bone-cement interface, along with non-linear and non-homogeneous, as well as post-yield properties of bone were taken into account in the FE models. The accuracy and precision of FE models in predicting bone strength were evaluated using statistical tests by comparing the FE results with the results obtained using in-vitro mechanical tests on 14 cadaveric specimens and an independent data set.ResultsAccording to paired t-test analysis, no significant difference was observed between the bone strengths predicted by FEM and those calculated by in-vitro tests . BackgroundThe most common method of treatment for vertical talus is the extensive soft tissue release. A minimally-invasive method based on Dobb's serial casting (Reverse Ponseti) was introduced 15 years ago and has yielded significant results. The aim of this study was to investigate and compares the midterm results of Casting with the extensive soft tissue release method.
Methods12 patients with vertical talus were included in a prospective study, 6 patients with 10 feet were treated with reverse ponseti, and 6 other patients with 10 feet with externsive soft tissue release. In an 18-24 months follow-up the results were compared together, using ankle range of motion, oxford questionnaire, and radiographic measurements.
ResultsThe mean range of ankle motion was comparable in the two groups (p=0.35), the oxford questionnaire score was not statistically different between the two groups (p=0.876).
ConclusionWith the comparable results between the ''reverse-ponseti'' casting land extensive surgical releases in this mid-term study, the casting technique is recommended because of its lower cost and non-invasive nature. rtical talus (CVT) is an uncommon congenital foot deformity. Incidence rate of CVT is 1 out of 10,000 live birth. CVT is one of the main causes of rigid flatfoot in newborns (1). Rigid talonavicular joint dislocation is the hallmark (inflexible in the sole of the foot with no arch) of CVT, accompanied by non-flexible equinus. In this structural abnormality, in fact, the convex plantar surface of the foot creates a rocker-bottom appearance (2). It occurs as an isolated deformity in half of patients and is associated with neuromuscular and genetic disorders in others (1, 3, 4).There is bilateral involvement in half of CVT patients, and there is no gender predilection (5). Although CVT is not painful for newborns and infants, if not diagnosed and left untreated, leads to major disabilities in the adulthood. Ambulation is not usually delayed; however, the gait is unbalanced. Patient`s shoes often show abnormal wear, and pain often develops in early adolescence (6). Pages 256-260BackgroundOne of the common reasons for referral to healthcare centers are orthopedic injuries which may occur due to spontaneous pathologic injuries, traffic accidents, workplace injuries, and during exercise. This study aimed to determine the frequency, types and causes of orthopedic injuries refering to Akhatr teaching hospital in Tehran.MethodsIn this study, records of all the patients presented to the emergency department of the Akhtar hospital in 2015 were reviewed. Data were collected by random completion of the "damage record questionnaire" by nurses. Data were analyzed using descriptive (frequency, mean and standard deviation) and inferential (Chi-square) statistics to evaluate the incidence rate of injuries and demographic characteristics of the participants and to compare the incidence rate of injuries in various body parts.ResultsMost injuries occurred in lower extremities, followed by upper extremities. In addition, there was a significant association between gender and cause of incidents (P<0.03). Moreover, a significant correlation was observed between gender and rate of injury in left (P<0.001) and right (P<0.049) lower extremities and also in the right (P<0.001) and left (P<0.001) upper extremities. Results demonstrated a higher rate of injury among men, compared to women. Furthermore, the ankle was the most commonly injured area in the lower extremities while the wrist was the most common area of injury in upper extremities.ConclusionAccording to our results, the main cause of orthopedic injuries in the emergency department was traffic accidents (drivers, car occupants, and pedestrians), and the majority of patients were in the age range of 20- 40 years.Keywords: Orthopedic Injuries, Fracture, Emergency

Pages 261-264Periprosthetic fractures about the humerus are a rare but difficult complication of shoulder arthroplasty, occurring in up to 3% of cases. Depending on the fracture pattern, open reduction and internal fixation (ORIF) may be used to definitively treat these injuries. This article demonstrates a new technique that uses a distal femoral locking plate to maximize bony fixation of such periprosthetic humeral fractures. Between November 2008 and July 2016, three patients with periprosthetic humerus fractures were treated with open reduction internal fixation using a distal femoral locking plate. All patients were followed until radiographic :union:. Fractures occurred at a mean of 5.2 years after arthroplasty (range, 1 to 7.5 years). Two of the fractures were classified as Wright-Cofield Type C and one fracture was Wright-Cofield Type B. Surgical fixation was performed at a mean of 6 days after fracture. All were treated with a fixed angle distal femoral locking plate. Healing was confirmed radiographically for all three patients at a mean of 11.8 months (range 3 to 28.5 months). No patient required reoperation. Obtaining adequate bony fixation around periprosthetic humerus fractures remains challenging. The use of a distal femoral locking plate may be helpful in patients with long stem prostheses which require increased distal fixation.
